Click the button below to fill out the required patient intake paperwork for your initial appointment. Please complete this paperwork as soon as you can after scheduling your appointment.
We will verify you have it completed during our appointment reminder call, the day before your initial appointment. If you do not complete your paperwork in advance, the patient care coordinator may need to adjust your arrival time to an earlier time, to allow ample time to complete paperwork on the day of your appointment.
All minors will need their parent or guardian signature on the appropriate paperwork.
Please remember to bring the following with you to your initial appointment:
- Your driver’s license or photo ID
- Your insurance card(s)
- Military ID (if Tricare is your insurer)
- VA ID (if VA is your insurer)
- Your prescription – if you have one
- Your completed paperwork (linked via button below)
- Your calendar
- We will schedule your entire plan of care at the appointment, so a calendar is required.
